Membership Reduced Price Request Change or Renewal

MakeHaven is committed to creating an inclusive community by offering sliding-scale memberships to those with financial need. This form is used to determine eligibility based on household income and, in rare cases, exceptional circumstances.

Program Highlights:

  • Sliding-scale rates are based on household income before taxes, as defined by how taxes are filed.
  • Applicants may qualify under certain exceptional circumstances (e.g., medical hardship or disaster recovery).
  • Documentation, such as tax returns, may be required to verify eligibility.
  • Membership renewals occur every 6 months, and requalification is necessary to maintain sliding-scale rates.

Sliding Scale Transitional Price Policy Plan

Funding for reduced-price sliding-scale memberships was generously supported in 2024 by a gift from the Achelis & Bodman Foundation and other individual donors. We are deeply grateful for their support, which made our initial program possible. Unfortunately, demand for reduced-price memberships has significantly increased, and no new grant has been identified to continue the previous level of subsidy. As a result, we have established updated sliding-scale payment levels, which are already applicable to all new incoming members.


New Monthly Sliding-Scale Prices

  • Living Wage: $50
  • 200% Federal Poverty Level: $40
  • Federal Poverty Level: $30
  • All Other Considerations: $30–$50 (minimum $30, based on specific non-income circumstances).

How This Impacts Existing Sliding-Scale Participants

While changes to the program are financially necessary, we are committed to maintaining inclusivity and minimizing the impact on current members. To achieve this, we have developed a transition plan that honors all existing renewal commitments while gradually implementing the updated rates.


Price Transition Policy

All sliding scale participants must complete the renewal form on their 6-month renewal point. To prevent financial burden, non-renewal results in cancellation of membership until renewal is completed.

  1. Renewal Increases Starting April 1, 2025:
    • All new renewals will include a $10 increase which will go into effect on April 1, 2025.
    • Subsequent 6-month renewal periods will continue to include a $10 increase until the member’s rate aligns with the updated sliding-scale level for their income category (listed above).
  2. Income Verification Required:
    • Income-based renewals require members to select an income level based on the sliding-scale table.
    • Non-income-based considerations (e.g., extreme medical expenses) are only available for individuals with documented exceptional circumstances beyond income eligibility. Members without exceptional circumstances will not qualify for non-income-based considerations.
  3. Sliding Scale Pricing Cancellation if No Renewal Form is Submitted:
    • Renewals are currently every 6 months. Renewal reminders will be sent by email. Those who do not complete the renewal form will have the sliding scale pricing expire and the membership will automatically revert to the standard rate.
